Use our tools, forms or guidance in your organisation

We share openly because we want as many people as possible to be able to plan for the end of life.

We allow other organisations to use our tools, forms and resources for free, under an open license.

Brand our resources as your own

You can use our resources with your organisation’s own logo and branding, for free, under a Creative Commons license. This means you could give out our advance decision forms in your GP surgery, include our information on planning ahead as part of your charity booklets or host our online advance decision service as part of your website or app.

Our mission is to make sure everyone receives the information they need to be in control of their end-of-life decisions, regardless of who they turn to for support.

Rigorously tested and respected information

Using the correct wording when recording end-of-life decisions is vital to ensure those wishes will be heard, understood and respected when it matters most. We develop our resources with dying people, clinicians and legal professionals. They are trusted and used by NHS Trusts and national charities, endorsed by the British Medical Association and the Royal College of Nursing, and linked to by the NHS.

Whether end-of-life decision-making is a big or small part of your work, you can draw on our expertise. We have spent years developing our suite of information and support, which evolves based on the experiences of the thousands of people we support each year. Using our resources means you can benefit from this insight for free rather than start from scratch.

How it works

Our tools, forms and resources are available under a Creative Commons license that allows you to use and adapt them non-commercially. All we ask is that you credit us and sign a simple agreement.
