This means you could give out our advance decision forms in your GP surgery, include our information on planning ahead as part of your charity booklets or host our online advance decision service as part of your website or app.
Our mission is to make sure everyone receives the information they need to be in control of their end-of-life decisions, regardless of who they turn to for support.
Using the correct wording when recording end-of-life decisions is vital to ensure those wishes will be heard, understood and respected when it matters most. We develop our resources with dying people, clinicians and legal professionals. They are trusted and used by NHS Trusts and national charities, endorsed by the British Medical Association and the Royal College of Nursing, and linked to by the NHS.

We have spent years developing our suite of information and support, which evolves based on the experiences of the thousands of people we support each year. Using our resources means you can benefit from this insight for free rather than start from scratch.
