We support people to start honest conversations about death and dying, and record and revisit their wishes whenever they want to.

We specialise in helping people to make a living will (advance decision) and also provide information on:

• Do not resuscitate (DNR) forms
• Lasting power of attorney (LPA) for health and welfare
• Advance statements
• Making decisions for someone else

What can you not help me with?

We operate within the law and do not provide information about:

• how to end your life
• how to access an assisted death

We are a national charity supporting adults to plan for the end of life. All of our services are free. This includes:

• living will (advance decision) and advance statement forms
• phone-line support to complete planning ahead forms
• information for families and friends supporting a loved one at the end of life

Any adult can contact us for support. You do not need to use a solicitor to make any of the forms we provide.

Our nurse-led information line (0800 999 2434) is open Monday to Thursday, 11am – 3pm (closed on bank holidays).

If you call outside of these times you will hear a recorded message asking you to call back within our opening hours.

When you phone (0800 999 2434) you’ll come through to a member of our team who may be able to help straight away. Depending on what you need help with, we may need to take your details and get back to you.

If you send us an email we aim to get back to you within three working days, not including bank holidays.

Callers ringing the information line can expect to receive a confidential service, creating a safe environment for you to speak freely. If you choose to share your details with us these will be kept confidential and used in line with our privacy policy.
