Advance Decisions to Refuse Treatment

An Advance Decision allows you to refuse treatment in advance of a time when you cannot make decisions for yourself. We provide free, legally binding Advance Decision forms, as well as giving practical support on how to complete them.

Health and Welfare Lasting Powers of Attorney (LPAs)

LPAs allow you to give one or more trusted people the legal power to make any decision about your health and welfare if you cannot make decisions for yourself. We can support you to complete and register your forms.

Leaflets and publications for individuals, carers and family members

We produce clear and up-to-date information on what you need to know to plan ahead for your future care and treatment, how decisions are made if you do not have capacity and a wide range of other topics around end-of-life care.

Information for healthcare professionals

We provide information on how to implement Advance Decisions and support patients to make their wishes known.



We provide one-to-one support to older people wishing to know and exercise their rights to plan ahead and make decisions about their end-of- life care.



We run information sessions and training for professionals, community groups and volunteers on a range of end-of-life topics, including accredited Continuing Professional Development (CPD) modules.



We conduct and review research into end-of-life issues to inform policy makers and promote patient-centred care.
