Every Moment Counts: a narrative for person centred coordinated care for people near the end of life, published today, sets out how good, coordinated – or integrated – care looks to people near the end of life and is written from their point of view.
We welcome the ‘Review of Choice in End of Life Care’ report findings and looks forward to patient choice becoming more integrated in healthcare, which has been lacking in end of life care in the UK.
One of the key messages that has come out for us from our pilot project has been around language. Words that we use everyday like end of life and palliative care, can be very scary for many of the older people we come in contact with or are words that are not fully understood or mean something quite different.
